What is Transgender hair transplant surgery?
Transgender hair transplant surgery is a common form of hair transplant that helps to provide trans patients with hair that matches the gender/sex they identify as. Hair transplants can be used to provide a more “feminine” or “masculine” hairline.
Hair transplants for transgender patients will often look to treat a condition called male pattern baldness (androgenetic alopecia) which causes hair loss in the temple and crown regions. This condition can affect MTF patients who have suffered male pattern baldness before their transition. It can also affect FTM patients who regularly inject testosterone as part of hormone replacement therapy.
In both instances, a transgender hair transplant would look to reshape the hairline and fill in the temple area. The aim here would be to achieve a result that enables the patient to have a hairline that is in keeping with their identified gender/sex.
At the clinic in Manchester, the main surgical method we offer for trans patients is the FUE (Follicular Unit Extraction) method. In some cases, the DHI or the FUT (Follicular Unit Transplantation) may be recommended for those who wish to keep their hair long after surgery if it’s possible.

Transgender Hair Transplant Cost
At the Manchester hair transplant clinic, hair transplants begin from £3,000. Typically, most transgender patients will pay between £4,500 and £6,500 depending on the size and severity of the patient’s hair loss before treatment.
The cost of a hair transplant is dependent upon several factors, namely the size of the surface area needing work, the complexity of the case, and the surgical procedure used. This information will be disclosed to you in your consultation notes after your initial consultation.
At the Manchester hair transplant clinic, we do not operate on a “cost per graft” basis like other hair transplant clinics. This is because no one can give an accurate number of grafts needed and extracted prior to surgery, so we keep things simple by surface area.

How Does a Transgender Hair Transplant Surgery Work?
Simply put, a hair transplant looks to take hair follicles from the donor area and implant these hairs into the recipient site. The donor area is found on the back and sides of the head. For MTF transgender patients, the recipient site is often in the crown and hairline area.
The predominant hair transplant method used for transgender patients is the FUE method. In some cases, patients can opt for the DHI method. Both of these hair restoration procedures extract and implant individual hair follicles, one by one.
Follicular Unit Extraction (FUE): Transgender Hair Transplant Surgery
The most common transgender hair transplant method at the Manchester clinic is the FUE method. We’ve outlined the process for you below.
On the morning of your hair transplant, you will have a consultation with your hair transplant surgeon. This will be to run through the events of the day as well as draw out your feminised hairline.
Extraction Phase
The first stage of a hair transplant is the “extraction phase”. This is where hairs are taken from your donor area. The donor area is located on the back and sides of a patient’s head. The hair transplant surgeon will extract your hair follicles evenly from the back and sides of your head to avoid any sign of noticeable over-extraction of the donor area.
For most transgender hair transplant surgeries, between 1,800 and 3,000 hair grafts are taken from the donor area. You will be provided with a full count of the number of grafts used at the end of the surgery.
The extraction phase usually takes between 2-3 hours and will be where your transplantable hair follicles are extracted and prepped for re-implantation. The extraction is performed by our surgeon and usually one technician. Another technician will usually be on hand to separate your grafts into “singles”, “doubles”, and “triples”.
After the extraction phase, there will usually be a short lunch break before the incision and implantation phases begin.
Incision Phase
The “incision phase” is performed by your hair transplant surgeon and prepares the recipient site for the “implantation phase”. During the “incision phase”, a series of small incisions are made across the recipient site.
For both MTF and FTM patients, the recipient site is usually located in the hairline and temple area. This is designed to alter the look of the hairline and provide a more masculine hairline for FTM patients and a more feminine hairline for MTF patients.
Implantation Phase
The final stage of a transgender hair transplant surgery is the “implantation phase”. This is where the hair grafts are carefully placed into the incision sites across the recipient area.
The main aim, here, is to implant the hair grafts evenly across the recipient area. “single” grafts are implanted towards the front of the recipient, helping to delicately shape the hairline. “doubles” and “triples” are placed behind the initial hairline to provide a thick and full-looking result.
Once this phase is complete, you will then speak to your surgeon about your aftercare.

Why Do MTF Patients Have a Hair Transplant?
The predominant reason male-to-female (MTF) patients wish to have a hair transplant is to counteract hair loss that has occurred pre-transition.
One of the most common causes of hair loss in cis men is a condition called male pattern baldness. This is where hair loss occurs in a patterned fashion in the temple and (later) crown areas. Even if a person transitions their gender and partakes in hormone replacement therapy, the hair that has been lost will not grow back.
In his instance, a transgender hair transplant performs two key functions. First, it looks to reverse any hair loss that has occurred in the temple and crown areas, providing a thick and full look. Second, the aim is to attain a natural-looking hairline that is feminine-looking.
By gaining a feminine-looking hairline, a transgender patient’s physical appearance closely matches the gender they identify with, reducing feelings of gender dysphoria and improving their self-confidence!
Why Do FTM Patients Have a Hair Transplant?
The predominant reason female-to-male (FTM) patients choose to have hair transplants is to counteract hair loss symptoms that have occurred as a result of the transition process.
Once a transgender patient is in the process of transitioning from female to male, they must engage in hormone therapy that chemically alters the body’s hormone levels and develops more masculine or feminine features.
When an FTM patient begins hormone therapy, they must inject male sex hormones such as testosterone into the body. While male sex hormones increase lean muscle mass, cause the shoulders to broaden, and increase libido, it can also cause a more masculine hairline to develop, simulating the condition of male pattern baldness.
As such, many FTM transgender patients come to our Manchester clinic looking for a hairline hair transplant that counteracts this condition. What a hair transplant also enables in this scenario is the opportunity to reshape the hairline so that it is masculine-looking.

Hair transplants are the only long-term solution to hair loss conditions such as male pattern hair loss and female pattern baldness. With the right aftercare routine, there is no reason why a hair transplant can’t last for a lifetime for transgender patients!
There is no evidence to suggest that it is dangerous for transgender patients to take finasteride. Finasteride is a useful aftercare medication as it helps to prevent hair thinning after surgery.
If you wish to take finasteride as part of your aftercare routine, be sure to consult your GP or speak to the hair transplant surgeon who has performed your surgery.
Please remember that finasteride is for MTF transgender hair transplants.
Eligibility for a hair transplant is always dependent upon two factors: the size of the recipient site, and the size and quality of hair follicles in the donor area. If you have enough donor hair to cover the recipient site, then you should be eligible for a hair transplant procedure!
