What is Transgender hair transplant surgery?

Transgender hair transplant surgery is a common form of hair transplant that helps to provide trans patients with hair that matches the gender/sex they identify as. Hair transplants can be used to provide a more “feminine” or “masculine” hairline.

Hair transplants for transgender patients will often look to treat a condition called male pattern baldness (androgenetic alopecia) which causes hair loss in the temple and crown regions. This condition can affect MTF patients who have suffered male pattern baldness before their transition. It can also affect FTM patients who regularly inject testosterone as part of hormone replacement therapy.

In both instances, a transgender hair transplant would look to reshape the hairline and fill in the temple area. The aim here would be to achieve a result that enables the patient to have a hairline that is in keeping with their identified gender/sex.

At the clinic in Manchester, the main surgical method we offer for trans patients is the FUE (Follicular Unit Extraction) method. In some cases, the DHI or the FUT (Follicular Unit Transplantation) may be recommended for those who wish to keep their hair long after surgery if it’s possible.

Transgender Hair Transplant Cost

At the Manchester hair transplant clinic, hair transplants begin from £3,000. Typically, most transgender patients will pay between £4,500 and £6,500 depending on the size and severity of the patient’s hair loss before treatment.

The cost of a hair transplant is dependent upon several factors, namely the size of the surface area needing work, the complexity of the case, and the surgical procedure used. Your patient advisor will provide a full personalised quote following your free consultation.

At the Manchester hair transplant clinic, we do not operate on a “cost per graft” basis like other hair transplant clinics. This is because no one can give an accurate number of grafts needed and extracted prior to surgery, so we keep things simple by surface area.

Hair Transplant Finance Plans

We also offer finance plans to help fund and spread the cost of a transgender hair transplant. See our hair transplant finance plan page for more info.

If finance is an option you wish to pursue, we would be happy to discuss this further. You can do this over the phone, or with during your free video consultation!

How Does a Transgender Hair Transplant Surgery Work?

Simply put, a hair transplant looks to take hair follicles from the donor area and implant these hairs into the recipient site. The donor area is found on the back and sides of the head. For MTF transgender patients, the recipient site is often in the crown and hairline area.

The predominant hair transplant method used for transgender patients is the FUE method. In some cases, patients can opt for the DHI method. Both of these hair restoration procedures extract and implant individual hair follicles, one by one.

FUE Transgender Hair Transplant Surgery

The most common transgender hair transplant method at the Manchester clinic is the FUE method. We’ve outlined the process for you below.

On the morning of your hair transplant, you will have a consultation with your hair transplant surgeon. This will be to run through the events of the day as well as draw out your feminised hairline.

Extraction Phase

The first stage of a hair transplant is the “extraction phase”. This is where hairs are taken from your donor area. The donor area is located on the back and sides of a patient’s head. The hair transplant surgeon will extract your hair follicles evenly from the back and sides of your head to avoid any sign of noticeable over-extraction of the donor area.

For most transgender hair transplant surgeries, between 1,800 and 3,000 hair grafts are taken from the donor area. You will be provided with a full count of the number of grafts used at the end of the surgery.

The extraction phase usually takes between 2-3 hours and will be where your transplantable hair follicles are extracted and prepped for re-implantation. The extraction is performed by our surgeon and usually one technician. Another technician will usually be on hand to separate your grafts into “singles”, “doubles”, and “triples”.

After the extraction phase, there will usually be a short lunch break before the incision and implantation phases begin.

Incision Phase

The “incision phase” is performed by your hair transplant surgeon and prepares the recipient site for the “implantation phase”. During the “incision phase”, a series of small incisions are made across the recipient site.

For both MTF and FTM patients, the recipient site is usually located in the hairline and temple area. This is designed to alter the look of the hairline and provide a more masculine hairline for FTM patients and a more feminine hairline for MTF patients.

Implantation Phase

The final stage of a transgender hair transplant surgery is the “implantation phase”. This is where the hair grafts are carefully placed into the incision sites across the recipient area.

The main aim, here, is to implant the hair grafts evenly across the recipient area. “single” grafts are implanted towards the front of the recipient, helping to delicately shape the hairline. “doubles” and “triples” are placed behind the initial hairline to provide a thick and full-looking result.

Once this phase is complete, you will then speak to your surgeon about your aftercare.

Why Get a Transgender Hair Transplant?

There are several reasons why transgender patients come to our clinic in Manchester enquiring about hair transplant procedures. However, the overwhelming reason many patients come to us is to alter their hair so their physical appearance matches their gender identity.

Whether you’re looking to feminise your hairline or reverse years of hair loss, hair restoration surgery is a surefire way to transform your appearance and improve your self-confidence. Begin your hair restoration journey with the Manchester hair transplant clinic by booking your free consultation!

Why Do MTF Patients Have a Hair Transplant?

The predominant reason male-to-female (MTF) patients wish to have a hair transplant is to counteract hair loss that has occurred pre-transition.

One of the most common causes of hair loss in cis men is a condition called male pattern baldness. This is where hair loss occurs in a patterned fashion in the temple and (later) crown areas. Even if a person transitions their gender and partakes in hormone replacement therapy, the hair that has been lost will not grow back.

In his instance, a transgender hair transplant performs two key functions. First, it looks to reverse any hair loss that has occurred in the temple and crown areas, providing a thick and full look. Second, the aim is to attain a natural-looking hairline that is feminine-looking.

By gaining a feminine-looking hairline through gender-affirming hair restoration, a transgender patient’s physical appearance more closely matches the gender they identify with, reducing feelings of gender dysphoria and improving their self-confidence. A feminising hairline transplant is one of the most impactful and long-lasting steps in the physical transition journey.

Why Do FTM Patients Have a Hair Transplant?

The predominant reason female-to-male (FTM) patients choose to have hair transplants is to counteract hair loss symptoms that have occurred as a result of the transition process.

Once a transgender patient is in the process of transitioning from female to male, they must engage in hormone therapy that chemically alters the body’s hormone levels and develops more masculine or feminine features.

When an FTM patient begins hormone therapy, they must inject male sex hormones such as testosterone into the body. While male sex hormones increase lean muscle mass, cause the shoulders to broaden, and increase libido, it can also cause a more masculine hairline to develop, simulating the condition of male pattern baldness.

As such, many FTM transgender patients come to our Manchester clinic looking for a hairline hair transplant that counteracts this condition. What a hair transplant also enables in this scenario is the opportunity to reshape the hairline so that it is masculine-looking.

In a similar way to MTF patients, hair transplants for FTM patients help to align the appearance of the body with a person’s gender identity. In doing so, hair restoration treatments can be used to improve a transgender person’s self-confidence and body image!

MTF and FTM hair transplants: What's the Difference?

The surgical procedure used for transgender hair transplantation is the same, regardless of the patient’s sex/gender. The only difference will be the intended aim for surgery, and the look of the result.

As part of a person’s transition from male-to-female (MTF) or female-to-male (FTM), transgender patients come to us at the Manchester hair transplant clinic wanting to obtain a physical appearance that is befitting of the gender they identify with.

In this instance, hair transplantation is used to “masculinise” or “feminise” a person’s hair, as well as reversing any signs of pre- or post-transition hair loss, thus improving self-confidence.

Another procedure offered is a beard transplant. Thanks to technological advancements with FUE, in which hairs can be individually extracted, beard transplants can also be offered. If you’re looking for a desirable beard, then a beard transplant may be for you! To learn about the process, please consult our beard transplant page.

Transgender Hair Transplant Recovery Timeline

Recovery from a transgender hair transplant follows the same general pattern as other FUE procedures. Because the recipient area is typically the hairline and temples, healing is usually well tolerated:

  • Days 1–3: Redness and minor swelling around the hairline and forehead — normal and settles within a few days
  • Days 3–10: Small scabs form around each implanted graft — leave these to fall away naturally
  • Weeks 2–4: Transplanted hairs shed — a normal part of the growth cycle, not a sign of failure
  • Months 2–4: Shedding ends; new growth begins to emerge from the implanted follicles
  • Months 4–8: Visible regrowth accelerates; hair thickens and texture normalises
  • Months 12–14: Full results visible — the transplanted hairline is permanent and grows as natural hair

FAQs

Hair transplants are the only long-term solution to hair loss conditions such as male pattern hair loss and female pattern baldness. With the right aftercare routine, there is no reason why a hair transplant can’t last for a lifetime for transgender patients!

There is no evidence to suggest that it is dangerous for transgender patients to take finasteride. Finasteride is a useful aftercare medication as it helps to prevent hair thinning after surgery.

If you wish to take finasteride as part of your aftercare routine, be sure to consult your GP or speak to the hair transplant surgeon who has performed your surgery.

Please remember that finasteride is for MTF transgender hair transplants.

Eligibility for a hair transplant is always dependent upon two factors: the size of the recipient site, and the size and quality of hair follicles in the donor area. If you have enough donor hair to cover the recipient site, then you should be eligible for a hair transplant procedure!

At the Manchester clinic we have 2 surgeons:

  1. Dr. Aziz Elgindi (GMC: 7450896). Dr Elgindi is the most experienced surgeon and has many years in performing both FUE and FUT hair transplant surgeries.
  2. Dr. Mohammed Ditta (GMC No: 7084372). Dr Ditta is trained in performing FUE hair transplants. He has years of experience and has performed a large number of surgeries.

No, hair transplants do not hurt. Before your procedure, you will be given a local anaesthetic on the scalp area. This will help to numb the scalp area, making the experience more pleasurable for the patient.

The number of grafts depends on the extent of hair loss and the goals of surgery. For MTF patients restoring a feminine hairline from a receded position, 1,800–3,000 grafts are typically required. For FTM patients seeking a more masculine hairline shape, the graft count is usually lower. Your surgeon will assess your donor density and recipient area at your free consultation to give you an accurate estimate.

Recovery is generally straightforward. Most patients return to desk-based work within 5–7 days. Scabbing settles within 10 days, and the shedding phase — where transplanted hairs temporarily fall out — ends by weeks 3–4. This is normal and does not mean the transplant has failed. New growth begins from month 3–4, with full results typically visible at 12–14 months.

Patient Testimonials

Aside from achieving fantastic results, we believe that keeping in touch with our patients before, during and after their hair transplantation procedure is paramount – it helps to keep our patients feeling calm and in control. We’re always on hand to provide guidance, support and aftercare advice. Time and again, our patients tell us that this is what sets us apart from other clinics.

It was an outstanding experience from start to finish. Naturally, I was unsure on which organisation to choose for the procedure, but HSHTC came across as professional and caring, as well as having a wide range of availability. There are negative reviews on all businesses, and this drives customers like myself away. However, something told me this would likely be someone with unrealistic expectations, or not reading into the procedure as a whole. I chose to ignore the negatives and read the positives. Sara the coordinator was absolutely amazing! She outlined everything and made it clear of what is expected and what is not. After a great consultation, I was passed to the next stage of meeting surgeon Dr Ditta. He presented extremely knowledgeable and passionate about what he does, so I decided to go ahead with the procedure. The surgery day was expected to be a long, but rewarding day. It started with having to shave my head (mandatory requirement for this procedure). I’d say to anyone the thought is worser than the event, and to just do it for the positives that will follow. Dr Ditta and his team were extremely caring and sensitive, especially with briefing before any painful situations pre anaesthetic. After that, I was allowed to top up when requested and leave them to do what they do best. The day as a whole was enjoyable and interesting to speak to the team to learn more about the procedure and get to know them. Lunch was provided (yay). It was lovely of them to offer, and you definitely need to eat mid procedure to keep energy levels. At the end of the day, I was given clear instruction of how to have the best success rate. It was through a video and in person. If you have questions, ask as it’s what the team is there for. The first few days of recovery can be quite awkward and uncomfortable, but I’d recommend booking 2 weeks off work to fully recover and let the grafts set. Once you get use to the sleeping situation, it’s easy and a lovely break from reality (put pillows below your knees and stack below head for pure comfort!). The aftercare products was extremely useful and easy to use. The grafts will fall out after a few weeks, it’s expected and you’re told this plenty of times!! Having a healthy lifestyle makes your hair grow quicker and thicker, so swap your diet to good healthy hair foods. When your transplanted hair grows back, it’s an amazing feeling, but only the beginning. As the months pass by, you’ll notice a massive improvement. During this process, you’ll have regular aftercare and the chance to raise any concerns. I personally didn’t have any issues and was just grateful to still be in contact and told everything is going as it should. I’m 6 months post surgery and I’m seeing the benefits hugely, and it’s not even reached maximum results yet, as this is one year. Thanks for an amazing service!

Having given some time and thought about having a Hair Transplant, I contacted Harley street hair transplant clinics. The consultant (Sara) explained everything regarding the procedure with Dr.Ditta and the technicians. From the beginning, Dr.Ditta explained the days events and how the procedures would be done beginning with taking a photo of my head and showing me where the grafts would be taken from and where they would be placed. The procedures would be done and completed in two parts, with a lunch break in between. Once completed, Dr.Ditta went through the aftercare, explaining what to do each day for 10 days. I was given capsules, shampoo, sponge, neck pillow for sleeping with and information regarding how to look after my hair. Dr. Ditta and his team were welcoming and friendly ensuring I was looked after throughout both parts of the procedure. Sara maintained regular contact to make sure recovery was going well. The results were great, patience is needed as the grafts start to grow, and then you can see the results. Extremely happy with everything, from start to finish.

Overall, my experience at the clinic was positive. Dr. Ditta was exceptional — he made me feel comfortable and at ease throughout the procedure, and his professionalism was truly reassuring. However, I was disappointed with one member of the team who, in my opinion, did not fully follow health and safety protocols. She had her hair down without a surgical cap and was using her phone while wearing gloves, then proceeded to touch my hair without changing them. This raised concerns about hygiene and cross-contamination. Her manner was also not very pleasant, which made the experience less comfortable. Although I was initially worried about the risk of infection, I felt more at ease by the end of my visit. I hope the management reviews this matter to ensure that strict hygiene and professional standards are consistently maintained. Aside from this, I was very satisfied with Dr. Ditta’s care and look forward to seeing my results.

Dear Team. Following my hair transplant last Saturday I would like to take this opportunity to thank all of the team, for making my journey as pleasant as possible. From the first consultation, to having my hotel booked, to the procedure itself, I couldn’t have been more impressed. Finally a big thank you to Doctor Ditta and the team, who were so kind when I felt unwell, making me drinking chocolate and reassuring me throughout the process. Add to this, their expertise and professionalism, what more can one ask for? I look forward to the final part of my hair transplant next year. Once again a big thanks to you all. Kind regards Keith Yates

Before the procedure I had online and face to face consultations with no obligations. These gave me all the information I needed and really put me at ease and gave me the confidence to proceed. Everyone was really knowledgeable and friendly and clearly very experienced. Booking the procedure was quick and easy and I got a date that suited me. The procedure itself went really smoothly I was able to put headphones in and listen to podcasts and was very comfortable. Afterwards I was given really detailed notes on what to do and what would happen and everything went exactly as planned. It was so reassuring knowing that advice was only a text message away and responses were always immediate. I'm two weeks post procedure now and everything has gone back to normal and the recovery is going just as planned. I'm so glad I did this and chose Harley Street Hair Transplant!

Great clinic with friendly and experienced team. I wanted my surgery done in the UK and found the clinic on Google. Process started with virtual consultations with patient advisor and then with the surgeon. FULL costs were given up front, with good finance options available. On the day I designed a new hairline with the surgeon and ended up getting about 1600 grafts iirc, which was good value for money. Clear pre- and post-op instructions given and recovery has gone as expected. Now waiting patiently for regrowth :) - Grant
